What is a Clinical Nurse Specialist?
A clinical nurse specialist is a registered nurse with a master’s degree in psychiatric nursing. Certified clinical nurse specialists have years of clinical experience working with psychiatric patients and their families, and have passed a certification exam. These providers draw from a medical and psychosocial knowledge base to provide patient care.

What is a Clinical Psychologist?
A Clinical Psychologist is a Ph.D or Psy.D practitioner with a doctorate in psychological testing and/or counseling. Psychodynamic, cognitive and behavioral techniques are used to achieve client wellness. Psychologists do not dispense medication.

What is a Clinical Psychiatrist?
A Clinical Psychiatrist is a medical doctor who is well versed in human physiology and disease management. Medications are used to re-equilibrate disordered brain chemistry and return patients to their desired level of cognitive and emotional functioning. Counseling and other therapeutic techniques may be utilized as well.

How do I go about refilling a prescription?
A. General
A prescription may be refilled by contacting your pharmacy. Once all the available refills have been used, the pharmacist will either call our office or send us a fax requesting a refill. We will process the request within 24 hours from the time it is received by our office and you will receive a confirmation call from our office indicating the status of your request.

B. Controlled Substances
Refills on controlled substances are typically done during an office visit. If a refill or a controlled substance is requested between office visits, and if it is a medication that cannot be telephoned into the pharmacy, there will be a $10 charge. To request a refill of this nature, the patient must call the nurse at 404-292-3810, ext.206 and state the patient’s full name, date of birth, and the requested medication. The patient will then be requested to pick up the prescription from the front desk at the time specified by the nurse.

Please take notice that numerous telephone calls to the office will only delay the processing of your request.

How do I go about scheduling or checking to see if I have an appointment?
If you are a new patient wishing to schedule an appointment with one of our providers, please contact our Front Desk Coordinator,  @ ext. 201. If you receive her voicemail please leave a detailed message and she will call you back ASAP.

If you are an existing patient wishing to change an appointment or check an appointment please contact Allyson @ 201, your provider at their specific extension, or the nurse @ ext.206.

Importantly, we provide each patient with a reminder card when they schedule an appointment while additionally providing a courtesy reminder call the night before a scheduled visit. This is, however, a courtesy and as such, any missed appointment or cancellation without at least 24 hours notice will be subject to a fee. Fees must be paid in full before scheduling another appointment unless that particular provider reasons otherwise.

What is PACT?
As our name implies, we form a PACT with each of our clients to work together to achieve our mutual therapeutic goals. We continuously strive to make each visit a satisfying experience. PACT (Psychiatric and Addictive Curative Therapies) offers experts in all areas of mental health including clinical psychiatry, psychological testing, professional counseling, and legal consultation. The PACT operates under an innovative, team approach where care is divided among specialties so our clients may receive the best and most comprehensive levels of care current mental health treatments allow for.

How do I go about requesting medical records?
Due to HIPPA regulations, medical records will only be processed with a signed authorization on file.

If a patient requests for records to be sent to PACT Atlanta, LLC they should contact the office that contains their records and request that they be sent here. We do accept records request via fax (404-292-3810) as well as by mail.

If a patient requests for records to be sent from PACT Atlanta, LLC to another office, they should sign an “authorization to release medical records”, which is available at the front desk or by contacting the medical records custodian, Angelena Ledford, at 404-292-3810, ext. 212, and she will process your request within 48 hrs from the time the authorization was received. Records may then be faxed or mailed to the requested party.

Notice: Due to the nature and sensitivity of psychiatric records, we are not obliged to release records to patients. We will gladly release records to other providers for continuity of care, disability agencies, attorneys, and other responsible agents as dictated by law.

Who do I talk to if I have a question regarding a bill?
If you have received a bill and have questions concerning its nature please contact the Billing Manager, Lily Faron @ ext.202, Shayla Clark, the Billing Specialist @ ext.218, or Angelena Ledford @ ext.212. We also suggest that you carefully read over the explanation of benefit provided by your insurance company for the date of service at our office. Most times, these statements provide the information that has led to your being billed (i.e. different copay, deductible, services not covered, etc.)

Who do I send my comments about the practice too?
As a proactive healthcare organization, our patients’ comments are very important to us. If you should ever wish to provide feedback please contact the office manager, Suzette White at ext.200 or send her a descriptive email explaining your experience.

What do I do in case of a medical emergency (i.e. adverse reaction to medication)?
If you have a life-threatening emergency you should immediately call 911 or travel to the nearest emergency room.

Our office is open Monday through Friday, 9:00 a.m. until 5:00 p.m. and is available for assistance throughout the day. If you have a problem that is non-life-threatening during normal business hours please contact the office and press “o” to get an available staff member to help you as soon as possible.

If you have a problem after normal business hours or during the weekend, there is a doctor on call at all times. To speak with the doctor on call, one would contact the office and listen to a voice recording that states how to contact the on-call physician.

How do I get samples?
Samples can only be obtained by calling the office 24 hours in advance. Samples will not be given to patients who “drop by” the office without notice.

Patient’s should call the nurse at 404-292-3810, ext.206 and state the requested samples. The nurse will return the call in a timely fashion and indicate when the samples may be picked up. Samples will not be given to patients who have not received a return call from the office.

Where are we located?
PACT Atlanta, LLC is located in suite 221 of the office complex located at 465 Winn Way, Decatur, GA 30030. We are conveniently situated just past Dekalb Medical Center off N. Decatur Rd. Below you can find some general directions or you may link to mapquest.com for a more specific itinerary.

From Suwanee or Athens:
Take 1-85 South to 1-285 East
Take Exit 39-A (which is the 4th exit) US 78 (Decatur/Atlanta)
There will be 4 lanes, stay in the left lanes, these will merge into Lawrenceville Hwy
Go to the 2nd light and turn left on Dekalb Industrial Way
Go to the 1st light and turn right on North Decatur Rd.
Go to the 1st light and turn left on Winn Way
Go about .3 miles and on the left hand side will be a sign that says 465
We are located in the upper level of the beige and red building

From Ponce De Leon:
Take Ponce East to Scott Boulevard
Turn left on Scott Boulevard, then turn right on North Decatur
Pass Suburban Plaza on your right. Cross over Church Street
Pass Dekalb Medical Center on the right.
At the 1st light across from the CVS Pharmacy turn right.
Go about .3 miles and on the left hand side will be a sign that says 465
